If you are interested in taking a class, please download the following registration form in PDF format and submit to Shelimah Productions with payment. Cheques should be made payable to D. Bangerter and mailed to 221 Galbraith Blvd., Milton, ON  L9T 5Y2.

**Please note:  Your Health Screening and Waiver form MUST be submitted before class. This form can be downloaded and printed at the link below.

Cancellation Policy

Because this session is planned on pre-registration numbers, there is no refund on deposits or payments, unless the class itself is cancelled due to lack of enrollment.  NO REFUNDS FOR MISSED CLASSES.  NOTE:  $10.00 service charge on all NSF checks.

*All students must complete and sign a Health Screening Form prior to class commencement.
